Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries, and areas within countries, that you are visiting to determine what vaccinations are required.

In order for you to get the best from our travel service we request that you complete a questionnaire at least 8 weeks before your departure date. A seperate questionnaire must be completed for each person travelling (including children). Please return the questionnaire(s) to the surgery as soon as possible.
Your travel plans will be reviewed by a Nurse. You will then be contacted by the Nurse, or a Receptionist, to arrange an appointment in our travel clinic. You may need more than one appointment.
If you are travelling at short notice you should contact the Reception Staff who will advise if it is possible to complete you vaccine schedule prior to your departure. If it is not possible, you may need to book with a private travel clinic.

Please remember to complete one questionnaire for each person travelling.
