Royal Agricultural College On-Line Registration

Before proceeding to complete the on-line registration screen, please read the document called ‘Instructions for completing on-line registration forms’.

Remember you must attend medical registration on Monday October 3rd to sign the form.

You also need to complete a health questionnaire as it may be some time before your medical records arrive at the practice.

You should print this off, complete it at home and bring it to medical registration.

Medical registration involves attending a designated room at college where a team from the Phoenix Surgery, led by your college nurse, will have your registration form ready for signature and will take in your health questionnaire.

The college nurse will be available at medical registration to discuss any health issues which you feel are immediately important. You can also see her during surgery opening hours as listed in the information entitled ‘Medical services at the RAC’.

It is a college requirement for you to be appropriately registered with the college doctor and it is important to complete the medical registration process on registration day.
